4. Appointment Scheduling, Treatment & Payment Terms
Scheduling appointments is contingent on availability. You agree to provide accurate and complete information when booking. You authorize us to bill you, your insurance, or other payors for the services you receive. As a client, you are responsible for paying any co-payments, deductibles, or fees not covered by your insurance. Cancellations or rescheduling may be subject to our cancellation policy (please refer to our separate cancellation/rescheduling policy). We reserve the right to refuse or suspend services to any patient who fails to meet their payment or appointment responsibilities.
5. Intellectual Property
All content on the Site (text, graphics, logos, images, videos, software) is our property or that of our licensors and is protected by copyright, trademark, and other laws. You may view, download, or print one copy of the content for your personal, non-commercial use. Any other use of the content without our express written permission is prohibited.
